Rather than navigating the traditional healthcare/insurance-based model for routine primary care, your membership supports an ongoing relationship focused on prevention, wellness, acute concerns, chronic health management, education, and long-term health goals.

Enrollment Fee $150 which includes comprehensive health visit, review of medical history, ordering/review of labs, and development of treatment plan.

White Glove Essential

Not included: Essential provides comprehensive primary/preventative care and communication with your White Glove provider. This tier is ideal for children, youn g adults, and patients who don’t require routine visits for healthcare conditions with a provider, but would like to maintain a trusted relationship with care when you need it.
White Glove Signature

Not included: Everything included in the Essentials tier plus extended appointments when appropriate, wellness planning, and savings on eligible White Glove services. This package includes 1 additional visit per membership year for no additional fee.
White Glove Elite

Not included: Everything included in the Signature tier plus quarterly visits instead of 1 additional visit. This tier is ideal for patients of any age who value routine health visits with your trusted provider as well as close communication regarding your health needs. It is also ideal, and recommended, for patients with chronic conditions that may require regular evaluation and close monitoring/follow-up and prefer to be in close contact with their provider.
